A double-blind, randomized cross-over study was conducted to assess the immediate effects of caffeine (150 mg)/herbal ephedra (Ma Huang; 20 mg ephedra alkaloids) compared to a placebo. A total of eight healthy participants (four males and four females) with an average age of 23.4 ± 0.8 years (mean ages for males and females: 25.3 ± 0.7 and 22.0 ± 0.7 years, respectively) and an average body fat percentage of 22.5 ± 3.1% (15.7 ± 1.2 and 27.6 ± 3.5% body fat for males and females, respectively) were enrolled in the study. The participants were moderate caffeine consumers, consuming approximately 150–300 mg/day.

The participants arrived at the laboratory after a 12-hour fast and following a caffeine-free diet for 48 hours. Prior to supplementation, resting energy expenditure was measured, and then every 30 minutes for three hours after supplementation, measurements were taken for 15 minutes each time. Heart rate and blood pressure were monitored every 15 minutes. Blood samples were collected every 30 minutes after REE measurement and analyzed for caffeine, ephedrine, free fatty acids, and glucose levels.

Long-Term Ma Huang Study

The aim of this study was to investigate the long-term safety and effectiveness of an herbal supplement containing Ma Huang and Kola nut for weight loss. The supplement consisted of 90/192 mg/day of ephedrine alkaloids and caffeine.

The study design was a randomized, double-blind, placebo-controlled trial conducted over a period of six months. A total of 167 participants with a body mass index (BMI) of 31.8±4.1 kg/m2 were randomly assigned to either the placebo group (n=84) or the herbal treatment group (n=83) at two outpatient weight control research units.

The primary outcomes measured in this study included changes in blood pressure, heart function, and body weight. Secondary variables such as body composition and metabolic changes were also assessed.

The results of the study showed that the herbal treatment group had a significant decrease in body weight (-5.3±5.0 kg) compared to the placebo group (-2.6±3.2 kg) (P<0.001). Furthermore, the herbal treatment group also exhibited a decrease in body fat (-4.3±3.3 kg) compared to the placebo group (-2.7±2.8 kg) (P=0.020). In terms of cholesterol levels, the herbal treatment group showed a decrease in LDL-cholesterol (-8±20 mg/dl) compared to the placebo group (0±17 mg/dl) (P=0.013), and an increase in HDL-cholesterol (+2.7±5.7 mg/dl) compared to the placebo group (-0.3±6.7 mg/dl) (P=0.004).

Although there were small changes in blood pressure variables (+3 to -5 mmHg) and an increase in heart rate (4±9 bpm) in the herbal treatment group, there was no significant increase in cardiac arrhythmias (P>0.05).

Overall, this study suggests that the herbal supplement containing Ma Huang and Kola nut may be effective in promoting weight loss and improving cholesterol levels. However, it is important to note that participants reported experiencing dryness as a side effect of the herbal treatment.
